However, an applicant for a dental intern permit or dental hygienist intern permit shall furnish the board satisfactory evidence that the applicant has been: (1) appointed to a dental or a dental hygiene internship in a hospital or similar institution operated under the laws of Indiana; or (2) employed as: (A) an instructor in a dental school recognized and approved by the Indiana dental board; or (B) a teacher or operator in a clinic in a public or parochial school, college, or university. (b) The fee for the permit shall be set by the board under section 13 of this chapter.
