Online applications are immediately sent to the Licensing Division for processing and don't have to go through additional departments for sorting and payment processing. \n\nIf an online application is available, you are strongly encouraged to apply online. \n\nTo apply for a new midwives license, submit the online application or submit a completed Midwife License Application Form (PDF) along with the $195 application fee and required documents. \n\nYou will have 1 year from the date the application was submitted to complete all licensing requirements.
